Your service

Experiences and progress

Every young person has a current plan written with them that says what they want to achieve.

You can show progress in education, training or work for each young person, with dates.

Health needs are known and met: GP and dentist registration, health checks and medicines records are up to date.

Young people are building skills for adult life (cooking, money, bills, appointments) and you record what they have learned.

Young people say they are listened to, and you can show changes you made because of what they told you.

Helped and protected

Staff know how to raise a safeguarding concern, and every referral is followed through to an outcome on record.

Missing from care episodes are reported in line with your protocol, and a return conversation is offered every time.

Risk assessments are current, written with the young person, and changed after every incident.

Allegations against staff go to the LADO, and notifiable events are sent to Ofsted on time.

Any physical intervention or restriction is recorded, reviewed by a manager and discussed with the young person.

Leadership and management

The manager reviews the quality of care regularly and has a written plan with dates to improve it.

Staff have regular supervision, and mandatory training is in date for everyone.

Recruitment files are complete: DBS, references, full work history with gaps explained, and right to work.

Independent visit findings (Reg 44 for homes) or your own monitoring findings become actions that are checked and closed.

You review the quality of the service at least every six months and keep the written review.
