Making It Real is a framework developed by Think Local Act Personal (TLAP) that describes what good, personalised, strengths-based care and support looks like, organised around six themes and expressed through “I” and “We” statements.
Making It Real was developed by Think Local Act Personal (TLAP) and is built around six themes that reflect what matters most to people who draw on care and support. Each theme has a number of “I” statements describing what good looks like from the individual’s perspective, paired with “We” statements describing what organisations commit to. It helps organisations that genuinely want to get better at personalisation to check their current practice, identify areas for change and plan action.
The six themes
- Wellbeing and independence — living the life I want, keeping safe and well, with support that promotes wellbeing and independence.
- Information and advice — having the information I need, when I need it, with parity of involvement between parties.
- Active and supportive communities — a whole-system approach that keeps me connected to the people and community that matter to me.
- Flexible and integrated care and support — multidisciplinary support that considers things from different angles and joins up around me.
- When things need to change — ensuring all involved or affected parties are part of decisions when my needs or circumstances change.
- Workforce — professionalism, empathy and respect from people who are skilled, supported and treat me and each other well.
How Making It Real is used
Making It Real provides a framework within which legal duties under the Care Act 2014 — including the duty to promote wellbeing — can be fulfilled. Providers use the “I” and “We” statements as a benchmark for co-production and personalisation, checking their practice against the statements and developing action plans to close the gap.
